If the rules are anything like classic Traveller, the "to hit" roll is about effective hits, since any hit does a minimum of 1 point of damage. Similarly a "missed" grapple doesn't have to mean you didn't grab the target, just that your hold isn't impeding them.

You might also consider that unexpected attacks should gain not only a to hit bonus but also a damage bonus.
