Way back when SPI published S&T magazine they had an issue that contained the game 'Dreadnought' w/ a companion article.
All 'generations' of BB were covered.
In the article they stated that the leading cause of BB destruction was 'accidental explosion'!
They posited that it was due to the fact that the bagged propellant for the guns became unstable over time & that, esp wrt the main guns, various navies neglected to 'turn over' the 'powder' in their magazines often enough &, therefore, the bags at the very bottom eventually became unstable enough to spontaneously ignite.

p.s. During WWII the japanese navy lost one of their  16" BB's when it just went & exploded while in harbor.
