Essentially all misjumps are the result of the crew knowing that the conditions for jump are unsafe, and then deciding to do it anyway because the risks from not jumping are worse than the risks from misjump. The few exceptions will be damage to the ship that is unknown to the crew (and any monitoring systems), or interruption at a critical moment during the process (by which time it is too late). In neither case would a nanny-AI help. - Tim
