In the TV Series of the Expanse, they suit up, and go to their stations on frigate style ship like the Rocinante. And we see that on most other war ships. I'd expect passengers to be confined to quarter and to buckle up, as well as getting onto vac suits. As for the decompression, in the first season of the Expanse, on the MCRN Donnager (a capital ship - so big that it fits two ships like the Rocinante), the main party is confined to some detention when the hull get pierced by a rail gun projectile. Beside the gruesome head blown of one of the character they have to react quickly to seal the bulkhead with a repair kit available: some material and a glue gun. They were not wearing vac suit. And still in a combat situation the rescue party managed to put up sealed walls to be able to access the still pressurized room and evacuate. A ship like that seems to have emergency suits at every corner. It ain't the HMS Titanic. Later in the show, with other situation, we clearly can see that any situation in combat including with "pirates" they wear suits. They protect against the environment (of course) but not against being killed by projectiles. It happen a few times. Note that they keep things pressurized it seems. I'm taking The Expanse as an example because it felt to me the most realistic of space combat situations, without huge hand waving like force fields, automatic helmets, etc. It represent a great source of inspiration on how to play space battles in a RPG.
