Anti-teleporter measures have been mentioned in some adventures, and
consist of putting obstacles like containers in open spaces where
teleporters might be expected to try and 'land'. This tells me that if
large anti-psi field generators exist they are not fitted to Imperial
warships for whatever reason (too expensive, too power-hungry, etc.).

As for personnel, CT Book3 gives the price of a psionic shield helmet as
Cr4000. That's a lot of money if it's general issue, so I would assume
they're only issued to officers, and probably only Navy and Marine (not
Army) in the Spinward Marches as part of their combat kit (so built into
marine armour and navy vacc suits). Imperial agents in the Marches would
wear them quite a bit, and elsewhere if it was thought their mission
would involve dealing with psions.

I don't think the Imperium generally tests service members for psionic
potential. Rather, people who were otherwise already trusted, or who
showed strong latent potential might be tested and recruited into those
institutes that were never disbanded but rather taken under Imperial
control. So might psions that were discovered if they seemed useful.
Those not so useful... well this is an entity that reserves whole
planets for use as prisons.
