I've always assumed you do nothing because otherwise rolling on a
difficult task becomes far more likely to mess things up than to
succeed, and that means players will resist having to roll for anything
that's not dead safe.

Also, an exceptional failure already has a penalty - the character needs
to make a determination check to try again without an increase in
difficulty.
