I can tell you that the US Navy recruits medical professionals to become naval officers. While there is a path for grunts to become MDs via the Uniformed Services University of the Health Sciences, most medical Corp personnel were enticed to join after becoming doctors dentists other specialized medical professionals and nurses.

teach him/her/it to be a doctor, or do they look for and recruit people who
have already trained to become doctors, and then teach them the basic
military skills that they will need in the service?
