You might want to consider buying one of the versions of the Twilight: 2000 rules (originally created by GDW but licensed to other companies for later versions). IIRC, it had fairly detailed rules for fallout and lingering radiation effects. The PCs were presumed to be adventuring among the ruins of a Central Europe which had experienced a Cold-War-turned-hot-WWIII, complete with tactical and limited-strategic nuclear exchanges.
