We have something similar here and I was able to track the information
from our original purchasing information.  I have since ordered
replacement folders and key guides from the following place:

        Ellis Systems Corp.
        Schiller Park, IL
        847-671-3100

They were originally ordered through Gaylord, I believe, but they no
longer carry them.
