I phoned the publisher on this one.  They asked me to fax them a copy of =
the cover, the title page, etc.  I received a very prompt reply.  Some, =
but not all, of the copies of _Qualitative Sociology_, vol. 21, no. 2 (=
summer 1998) were defective.  The publisher will replace the copies based =
on claims -- they are not planning to reprint and ship to all subscribers. =
