Check your October 16, 1997 issue of New England Journal of Medicine.  We
receive two copies (one at each campus of the medical center) and both
have pages 1107-1112 bound out of page sequence (pages 1105-1106 are
present, then it jumps to 1113-1114; pages 1107-1112 are bound AHEAD of
page 1105, and even then, >they< aren't in order (1111-1112, then
1107-1110)!
