I have received a reply from Charles Schlacks.  We are expected to tip in
the illustrations.  The first packet of prints go on pages 136-140, the
second set on pages 248-261, and the third set on pages 323-343.  The
illustrations are in order, and go one to a page, with one exception.  The
first two illustrations of the third packet are to be mounted on the same
page 323.  Mr. Schlacks states "Before I mailed this material to all the
subscribers, I checked with librarians in the Los Angeles area.  They
advised me NOT to attach the illustrations because every library  has a
different procedure."

Guess it's time to get out the glue and brushes!
