Hi Anne,
  You're not alone.  The 'no-longer free copy' information only came to
my attention about 2 weeks ago when someone wanted an article.
Subscriptions can be had from US GPO and the Massachusetts Medical
Society.  According to the person on the phone, they do sell back issues
and will backdate a subscription. As a result we are beginning our sub
from May 1996.  Their 1997 prices have increased (I don't remember
specifics) and I think they are slightly less than SupDocs.
