thanks to those who answered!

What we've found is that both MCOG & us bind our supplements in the
back of the volumes. My policy here has been to look at the
pagination, before binding supplements in the back of the volumes.

Unfortunately, not only was our v.87,no.5,pt.2, May supplement bound
at the back of the volume, it was in the v.88 volume! Oh well. We're
going to have to rebind both volumes now...

Without your help, I would never have found this item.. thanks for
the clues & for helping with this mystery hunt! Our patron thanks all
of you too...

What you are missing is volume 87, issue 5, part 2 (a supplement).
The pages do run from 749-97 and it is a series of articles about
autism.  We did not receive this either, but were able to get it
through ILL.  Since this was before my time in Serials, I don't
remember who we got it from, and we no longer have those records.
Hope this is of some help.

Kimberly, I just checked our issue here at the College of Physicians of
Philadelphia.  The article(s) on autistic children appear(s) in the
supplement to the May issue of v.87.  The publisher labelled it as
v.87, #5, pt.2, May 1991.

a Supplement to Pediatrics numbered volume 87, number 5, part 2 for May

Kimberly - Our copy of Volume 87 of PEDIATRICS has all of the pages
you lack; they were originally published as a separate supplement,
which perhaps your library did not receive.  Supplements to this title
always page in.  (Hint for the future, though it won't help with your
present problem : before sending a volume to the bindery, double-check
to be sure that not only are the issues in correct numerical order, but that
there are no gaps in pagination.)  There are six separate articles included
in those pages; all of them are on autism.
      Unfortunately we are required to charge for articles that we send to
other institutions, unless we have a prior reciprocal agreement with
them.  The charge is $10.00 per article, payable in either New York
Academy of Medicine coupons or a check made payable to "The CHOP
Medical Library."
       Generally pre-payment is required, but if you are in a rush, you can
fax the request and we can fax back the article at no additional charge,
with the expectation that you would  mail payment upon receipt of the
article. If you were to send us a standard ALA interlibrary loan request
for a particular citation, we could fill the request promptly.  Our fax
number is 215/590-1470.
       Of course, if your patron wants all six articles, you would no doubt
want to try a less expensive way of obtaining the material.  So I'm not
sure how helpful I am being to you, except for telling you that the pages
exist.  If cost is not an issue, however, you may not have to look any
further.  (I've seen occasional patrons willing to pay what seem to me
rather fantastic fees to obtain certain articles quickly from other libraries,
but I know this varies greatly with the individual.)
