Michael, I'm not ready with evidence of omission, because all I have
to offer are impressions.  But I think your assumption is quite
justified.  As is all information that would be added to a record after
first cataloging, information about cumulative indexes is
likely to be missing (as are additional or changed issuing bodies,
publishers, bibliographic relationships, etc.)  CONSER libraries, we all
know, make a conscientious effort to add such information to the
national record, but keep in mind that most libraries are *not*
capable of altering OCLC master records to add information such as an
index.

Perhaps you could find evidence of publication of indexes in vendor
databases and then compare to the OCLC record.
