We experienced the same problem with nursing journals.  We also had a
nursing student working for us at the time who was able to pinpoint
the class that probably represented the source of the problem.  Our
nursing bibliographer simply contacted the faculty member, informed her
of the seriousness of the problem, and it stopped.  Obviously, the
culprits are not always so easily identifiable, but when they are, I
think that most faculty members would appreciate being involved.
