I read a note about this yesterday in the NLM Technical Bulletin,
July/August 1995, p.4 (it just happened to still be on my desk).  It says:

"In June [1995], the NLM
received notification from the BMJ Publishing Group that as of January
1996, the British Heart Journal will change its name to Heart.  The ISSN
number of the retitled journal will be 1355-6037.  The journal cover will
indicate that the title was formerly the British Heart Journal."
