We are dissatisfied with the claim postcard that we are currently using to
claim missing/lapsed issues of serials.  I was wondering if anyone who
feels that have a particularly good claim slip/form would be willing to
share it with us?  Also, any comments about what makes a good claim form
would be useful to us as we redesign ours.
