At UB, we based our decisions for VHLD on the ANSI standard, not LC
internal practice.  We use captions in the language of the piece,
abbreviated according to the list in the appendix of AACR2.  If there
is no caption we do not use one.(see 4.6.1.7 of the standard)
We are using Z39.44-1986 as our guide.  We are presently working our
finalizing our internal documents to account for all the unusual and
special cases.
