A call to the Circulation Department of the publisher (800-523-1546) this
morning elicited the report that library subscribers will be automatically
sent the author & subject index to v.113 of the Annals of internal medicine.
The subscriber's card does not need to be sent in to the office.  Libraries
should receive the index sometime in the next two weeks.

I appreciate the alert, however.
