We are looking to implement TERMS review of e-resources practices.  If any of you use this method of evaluating your e-resources, would you have examples of online forms, templates used to document the selection process and the deselection process,  or workflow practices that you would be willing to share?

Also is there a standard or accepted verbiage when it comes to informing faculty that an e-resource is being evaluated for possible cancellation?
