With Volume 42, 2009, the publisher indicated that it was ONLY in the CD-Rom Format.   Just Recently, I found a letter (which I cannot find) stating that if you wanted one of a limited number of print copies, you should send back the CD-Rom.
(which I did)  I have not yet received the print format.

I think it would be better instead of a CD-Rom that they offered it as E ONLY (but may not be cost effective for them?)
