My take on the rules would say that this is a major change because the
meaning of the title has changed, i.e. "care" has changed to
"medicine".  Those considering it a minor change are probably counting
words and saying that this is a minor change because the new wording
comes after the first five words, but this rule doesn't apply if the
meaning of the title has changed.
