Hi Amy and Peter —
An excellent source for "all things serial" — particularly those pesky little changes in title/ISSN/publisher, etc., is Ulrich's Periodicals Directory ä, available online (free) at http://www.ulrichsweb.com/ulrichsweb/. To search, click on the "Advanced Search" button at the top center. Also, explore the site in general — it has some pretty nifty resources for serials information and tracking.
I took the liberty of copying and reformatting a portion of their entry for your title, see below. Hope this helps. You'll need to contact the publisher to make sure that your subscription remains 'intact.'
Hope this helps a bit.
Tara

I think that for my library, it was an automatic switchover... that is, Nursing homes stopped and Long-term living arrived in its place...
But I'll be the first to admit that I don't understand serials very well, so I could be wrong! ~Amy

If indeed ISSN 1061-4753 has ceased, what happens for those of us who
subscribe(d) to it and not to ISSN 1940-9958?? Will the publisher let us
know or place re-directs on the web site (okay, I'm probably dreaming
about the latter ....)? If anyone has any information, please share with
the list!!
Thanks,
