Yes.
How often?
Monthly.
Who processes the claims?  Do you
use a subscription agent?  Which one(s)?
As Serials Technician, I do. I run a list of over dues & missing from our ILS. I have student check it first for titles that are listed by our subscription agent (EBSCO) as publisher behind schedule. I also have the student check the shelves for issues listed as missing. I check the other titles to see when/if they were dispatched by the publisher on EBSCONET, the serials management web site provided by EBSCO. If the overdue item had been dispatched 30 days or more I can claim it on EBSCONET.
EBSCO also provides a claim checker report that I review monthly to keep track of which claims we have received.

Do you ever claim DIRECT from the publisher?
Yes for the few titles that we order direct (not through our agent).

If you do NOT claim missed issues, what procedures are in place in your library for this matter.
