Maybe things have changed but when I worked at law libraries (1990's), I
always went direct to the publisher for either missing pages or missed
releases. They were pretty prompt about sending the replacements.

One of the problems we are facing with our serials vendor is the
length of time it takes to fill a claim for missing loose-leaf
pages/updates.  Currency is of utmost importance in our
collections.  We had worked out a system with the vendor which
appears to be hit or miss.  Does anyone have a procedure/process they
use with serials vendors for claiming this type of material that works
well?
