Hi Kathryn and colleagues,

I have just graduated from MLIS through UBC and Dawn will be finishing in
April.  Our paper has been accepted for publication by "Against The Grain"
http://www.against-the-grain.com/.  We assure you that any information you
send us will be kept confidential (in my locked drawer at home) and that
we are not affiliated with any commercial organization.  The reason we
want to know of the name and size of the institution you serve is to see
if the size makes any difference in the choice of formats.

Are you sure your survey is for an academic paper?  It looks like an
attempt to generate a mailing list for some commercial purpose to me!
Why do you need so much specific and personally identifiable information?
Are you affiliated with an academic institution? I'm afraid I must decline
to participate on these terms.
