I remember receiving a notice that a format change was coming but I don't recall receiving a survey beforehand.

Also, I just spoke to my representative about our first invoice.  One of the problems on it is that the same series number repeats for each title listed; she told me that Blackwell's has already received a number of calls about this problem and that they are working to fix it.

Yes, I noticed it too.  I also don't like the lack of  information about
what we paid for.  I do not recall being sent any survey to evaluate it,
but I must admit I sometimes ignore all these surveys.
