We have faculty-like status, and must achieve continuing appointment after
several years, which is like tenure. Annual evaluations are first a
self-evaluation written by the evaluee, sort of what I did last year, then
an evaluation written by the supervisor, and comments and signatures up
the line (assistant dean, dean). We've used the same forms for a number of
years and see a need for revision. I think most of us think it is fair,
just not very representative of our jobs so many years later.

In addition to annual evaluations, a peer librarian promotion committee
evaluates librarians that must come up for promotion and continuing
appointment, and yet another peer committee evaluates all librarians with
continuing appointment, every year, to satisfy institution and state
government demands for "post tenure review." These go all the way up
through the provost and president to the board of regents.
