follow a check-in process for incoming serials
intended to generate claims for damaged and
missing numbers. Subscribers that have lost or
damaged their issues should take responsibility.

I know I've reclaimed an issue that had arrived and somehow snuck through
without being removed from (my vendor's online claim database). I also know
I've been told by a publisher that an issue I claimed had never been
published, while my claim for it was based upon the holdings of several
online library catalogues - which listed it (sometimes along with the date
received) plain as day.
