Modify as needed:
500   Imprint varies.
500   Vols. for Sept. 1999-Jan. 2001 published: New York :  Columbia
University Press ; vols. for Feb. 2001-    published: Boston, Mass. :
Harvard University Press.
500   Imprint varies: New York : Columbia University Press, c1999-
