Incomplete microfilm reproduction

Apologies if you see this twice, I'm posting it to both Autocat and
Serialst.
I am cataloging an incomplete microfilm reproduction of a serial. The
reproduction will probably never be complete, since the agency that had
the filming done couldn't find a complete run of the title. The first
three volumes and the first two issues of the fourth volume are not
included and correspondence indicates there are issues missing within
the run that was filmed.
Based on what I've found in the archives of various lists, the CONSER
documentation, AACR2R, and LCRIs, I am doing the following:
Dates in the fixed field reflect the dates of the original title.
Form in the fixed field is "r".
Added an 007 for the film.
300-362 reflects original title.
533 for the film includes a note: "Vol. 1, no. 1 to v. 4, no. 2
lacking."
Questions:
1. Is what I'm doing correct?
2. Since I don't know that issues in the filmed run are missing,
should I make a note? Since the correspondence indicates there may be
missing issues, should I make the note in the 533 more along the lines
of "Several issues lacking"?
