At the University of Southern California we don’t retain or bind print copies of theses or dissertations. These are submitted and stored electronically and made available via our Digital Library.
We considered the release of signature pages in such an open venue to be a potential security issue and so collect the originals and store them physically in our University Archive. Periodically a student or some other bona fide person needs to have a copy and we then scan the requested signature page to a PDF and forward it electronically to the requester.
We have discussed digitizing the signature pages and storing these centrally, not only to make filling requests easier for us, but also to ensure digital preservation of these in the long term. But thus far we have not set up the necessary electronic infrastructure to manage them in that way.

University of Kansas Libraries do not maintain bound copies of theses or dissertations---we maintain these on, Scholarworks, our institutional repository. We do however provide instructions to facilitate departments or individuals who want bound copies.
